I can't remember which exact version this feature was added into, but it is in the current version 9 release. You'll just need to update your survey definitions so that the survey links are selected on the incident fields tab. If you include both the rep and eud links the eud link will show for the customer who is filling the survey out and the rep link will show when one of your support team members is viewing a response.
